PKD is a genetic disease, which develops progressively. Some patients may
concurrently have polycystic liver and renal calculus. And renal calculus may
cause pain. Because of gene inherited from the parents, the epithelial cells
proliferate abnormally, yet apoptosis of this kind of cells is inhibited. So,
when proliferating to certain extent (saying the diameter exceeds some
millimeters), this proliferated part will be separated from the renal tubule or
collecting tubule, so an isolated cyst filled with liquid will form.
At this time, the phenotype of renal tubular epithelial cells changes, and
renal tubular epithelial cells turns into cyst-lining epithelial cells, which
can secrete more and more cystic fluid; however, the cystic fluid become more
and more and can not be discharged normally. With large amount of cystic fluid
accumulated, the cyst become larger and larger, and the pressure inside the cyst
also increases, which increases the surface tension of the cyst.
In this condition, some sensitive patients would feel discomfort at the lower
back. Well, when infection occurs at part of cyst, patients will feel pain. With
growth of cysts, normal renal structure will be changed by these cysts. You see,
glomeruli inside the kidney can selectively filtrate red blood cells and renal
tubules can concentrate the urine, reabsorb some useful substances filtrated by
glomeruli, and regulate water balance and other electrolytes inside the body,
which influences the blood volume, thus affecting the blood pressure. So, when
the glomeruli and renal tubules are damaged, their roles can not be played
perfectly, so high blood pressure, abnormality in urine test and renal function
test appear.
